A nursing home investigation in Milford, Pike County, Pennsylvania led to the hospitalization of 15 residents Tuesday afternoon following complaints of suspected medical neglect at Belle Reve Senior Living Center.

Pennsylvania State Police confirmed that an anonymous tip prompted a coordinated response at the Milford-based personal care home, which houses approximately 83 residents. Troop R’s Major Case Team, along with Pike County detectives and the Pike County Area Agency on Aging, conducted an on-site review.

Residents were transported to hospitals throughout the Northeast Pennsylvania region and neighboring states, including:

• Lehigh Valley Hospital (Pennsylvania)
• Bon Secours Community Hospital (Port Jervis, NY)
• Garnet Health Medical Center (Wallkill, NY)
• Newton Medical Center (New Jersey)

Because of the number of residents involved, Pike County Department of Public Safety, local fire departments, and EMS crews coordinated a multi-agency emergency response.

The Pennsylvania Department of Human Services (DHS), which regulates and licenses personal care homes across the Commonwealth, confirmed it responded directly to complaints involving the Belle Reve Senior Living Center facility.

“Last night, members of our team responded to complaints we received concerning Belle Reve Senior Living Center. While we are actively investigating those complaints, we cannot comment further on specific investigations due to patient privacy,” a DHS spokesperson stated.

Although details remain limited, confirmation of an active DHS investigation raises serious questions about conditions inside the Pike County memory care unit.

Facility leadership stated that the hospital transfers were precautionary and that Belle Reve remains fully operational. According to the facility, those transported were residents from the third-floor Memory Care unit. Several residents reportedly returned later that evening.

However, at least one Pike County family strongly disputed the facility’s characterization of resident safety. The family reported repeated hospitalizations since their mother’s admission in late 2025, alleging over-sedation, falls, and inadequate supervision. They indicated their loved one would not be returning to the Milford facility.
